EXTENDING APPOINTMENT AVAILABILITY
Following a successful trial of appointments available to book online, we are pleased to announce that all pre-bookable Doctors’ appointments from September will be available to book online from the 28th July 2014.
These appointments will also be available for booking over the telephone.
We would like to encourage our patients to use this service. If you haven’t registered, all you need to do is to come to Reception with photo ID and we will provide you with your password.
SHIP STREET SURGERY

Children Under 14
Please note that the Practice Nurses will not be able to see children aged under 14 without a parent or guardian present.
Children aged 14 – 16 years
The Practice Nurses will see unaccompanied children aged between 14-16 years provided there is letter of consent signed by the parent or guardian specifying the treatment to be given.

This information will be taken from the practice in a form that can identify you. The Practice has NO choice but to allow the HSCIC to extract this information. However, individual patients can instruct their Practice to stop the transfer of their data. Should you NOT wish your information to be shared, please notify the Surgery in writing either by letter or by using the opt out form.
