You can now order your repeat prescriptions or book appointments online.
To use SystmOnline you will need a Practice ID number and Access ID number which is obtained from the practice, and, in conjunction with a password, will be unique to you. Click on the link below to log in.
Your Hospital Appointment/Treatment
Once a referral has been sent, patients are under the care of the relevant Hospital.
Any issues or queries should be raised directly with the Hospital in question.

BLOOD PRESSURE MONITORING SERVICE
PLEASE REGISTER FOR SYSTMONLINE TO REQUEST YOUR PRESCRIPTION.
THIS SYSTEM REQUIRES A "ONE OFF" REGISTRATION WHICH IS DONE AT THE RECEPTION DESK.
YOU WILL NEED TO BRING A FORM OF PHOTO-ID SUCH AS A DRIVING LICENCE OR PASSPORT. ONCE REGISTERED, YOU WILL HAVE ACCESS TO OUR ONLINE APPOINTMENTS AND PRESCRIPTIONS.
Named Accountable GP for all Patients
You may be aware that from April 2015 all GP practices are required to provide their patients
with a named GP, who will have overall responsibility for the care and support
that our Practice provides to them.
If you wish to know who your named GP is,
please ask a member of staff.
Please note that this does NOT prevent you from seeing
any other GP in the Practice as you currently do.
Violent or Abusive Patients
The Staff have the right to work in a safe and secure environment and we as employers have the legal responsibility of providing that environment.
The Practice will not tolerate:
Verbal abuse to staff which prevents them from doing their job or makes them feel intimidated or unsafe
Threats of violence or actual violence to any member of the Practice
The GPs have the right to remove with immediate effect any patient who behaves in the above manner.

Children Under 14
Please note that the Practice Nurses will not be able to see children aged under 14 without a parent or guardian present.
Children aged 14 – 16 years
The Practice Nurses will see unaccompanied children aged between 14-16 years provided there is letter of consent signed by the parent or guardian specifying the treatment to be given.
Important Changes to how we handle your Personal Data.
We will soon be required to supply your personal and confidential medical Information to the Health and Social Care Information Centre (HSCIC). This information will be taken from the practice in a form that can identify you. The Practice has NO choice but to allow the HSCIC to extract this information. However, individual patients can instruct their Practice to stop the transfer of their data. For more information on the reasons for this data extract and how to opt out of sending your confidential information, please see the Notices at the Surgery and on our website under the CARE DATA tab. Should you NOT wish your information to be shared, please notify the Surgery in writing either by letter or by using the opt out form.

If you would be interested in using this service,
please bring photo-id (passport/driving licence) into the Surgery and
the Receptionist will print out your username and password for you to be able to
BOOK / CANCEL your appointments online.
If you have asked a local pharmacy to request medication on your behalf,
please be aware that this is a private arrangement between patient and pharmacy,
in which Ship Street Surgery has no part.
Please phone after 10am for test results.
This allows the Reception Team to deal with booking the urgent appointments and visits early in the morning.
